Other Community Team

  • Primary Care Network (PCN)
  • Clinical Pharmacist: Clinical pharmacists are integral members of the general practice team, working to enhance the value and outcomes of medication use while consulting with and treating patients directly. Their role includes providing additional support for managing long-term conditions, offering guidance to those on multiple medications, and improving access to health checks.
  • Pharmacy Technicians: Pharmacy Technicians support the delivery of safe, effective and efficient systems for medicines optimisation, repeat prescribing, reducing medicine waste and monitoring patients on high risk medication ensuring tests (mainly blood tests) are carried out regularly.
  • First Contact Physiotherapists: First Contact Physiotherapist (FCP) are experienced physiotherapists who have advanced skills necessary to assess, diagnose and recommend appropriate treatment or referral to musculoskeletal (MSK) problems on a patient’s first contact with healthcare services.
  • Mental Health Practitioner: Psychological wellbeing practitioners (PWPs) are trained to assess and support people with common mental health problems, principally anxiety disorders and depression, in the self management of their recovery.
  • Support Time and Recovery Worker: A Support Time and Recovery Worker provide support and give time to the patient to help their recovery.  Patients will usually be referred from the Mental Health Practitioner and will have been identified as requiring additional non-clinical support to manage a variety of issues, such as homelessness and substance misuse.
  • Social Prescriber Link Workers: Social Prescribers help reduce health inequalities by supporting people to unpick complex issues affecting their health and wellbeing. They connect people to community groups and statutory services for practical and emotional support.
  • Care Coordinator: Care Coordinators assist in navigating and coordinating care throughout the health and care system. They help patients access the appropriate care and support for their conditions by connecting them and their carers with the right services or GP staff to meet their needs.
  • General Practice Assistant: General Practice Assistants (GP Assistant) support doctors in the smooth running of the surgery by handling routing administration and some basis clinical duties enabling the GP to focus more on the patient.

Non-urgent advice: Medical Administrators

Medical Administrators serve as an essential link between patients and the practice, acting as the first point of contact for general enquiries. They provide basic information about services and results, and help direct patients to the appropriate person based on their health issues or queries.

Medical Administrators are responsible for scheduling most patient appointments with GPs and nurses using care navigation. They also undertake various important tasks, such as registering new patients, managing repeat prescriptions and related enquiries, handling financial claims, maintaining patient records, and conducting searches and practice audits. Additionally, they can generate referrals when needed.
