How to Register as a Patient
Register with us Online
Go to the Register with a GP Online Service (opens in a new browser window) and follow the on screen instructions.
Other Ways to Register
If you do not wish to Register Online you can download and complete the following form:
- Register with a GP Surgery (PRF1) form
- You do not need photo identification to register, but will need to provide this if you wish to set up online access to your medical record.
When you have completed the form visit reception in person with it.
Temporary Residents
The Doctors are happy to see patients who are temporarily resident in the Practice area should medical advice be required. In this instance persons wishing to be treated should state that they are a temporary resident when reporting to reception.
To register you will need to bring your passport and complete our Temporary Resident Registration form (also available from reception).
How to Find Your NHS Number
An NHS number is a 10 digit number, like 485 777 3456.
Your NHS number is unique to you. It helps healthcare staff and service providers identify you correctly and match your details to your health records.

New Registrations Health Check
You will be asked to make an appointment with the Health Care Assistant who will ask you one or two details about your past health and present life style. This will benefit both the practice and yourself as it can take some time for your notes to be received from your former GP.
Accountable Named GP for All Patients
When registering with Chadsmoor Medical Practice, you register with the practice, granting you the flexibility to consult with any of our clinicians. If you prefer to see a specific clinician, please inform us when booking your appointment. Kindly note that this may result in a longer waiting time. For continuity of care, all clinicians have access to your medical records.
